Ongoing monitoring

Keep four parts of the update together:

• Comparison basis: the specified prior state and the current state, including their source dates.
• Affected records: the asset or program identifiers touched by the change.
• What changed: the old and new values or conditions, with the reason for the update.
• Supporting sources: the references behind both states, retained with the review outcome.

Act on the change

Start with the prior state, inspect the affected assets or conditions, and open the supporting sources. Keep the change and the resulting review decision together so the next portfolio discussion can distinguish a new issue from one already considered.

Keep market inputs traceable

Retain the market reference, entity identity, and source date behind each input. Keep research inputs and buyer-issued terms as distinct parts of the investment record.
